NC*Notify is the event notification service of NC HealthConnex. This service provides near real-time admission, discharge and transfer (ADT) alerts on patients whom you choose to monitor.

Health care providers are often unaware when one of their patients has received care outside of their facility or EHR network. What’s more, there is a growing need for health care providers to be notified of certain events in a timely manner to support successful transitions of care and improve care management and meet repayment incentive requirements.

Full participants in NC HealthConnex can use NC*Notify to get significant insights into patients’ health care activity from more than 140 participating hospitals, plus encounter data from thousands of ambulatory care settings.

  1. A patient goes to the hospital or urgent care.
  2. The patient has x-rays and other tests.
  3. The hospital or urgent care sends information about the patient's visit to NC HealthConnex.
  4. NC HealthConnex notifies the patient's provider about the visit.
  5. The patient's provider makes a follow-up appointment for any additional care.

NC*Notify is a subscription-based service that notifies providers as their patients receive services across the care continuum. NC*Notify real-time event notifications provide care teams with valuable information that spans geographic and care settings and supports state and federal efforts to focus on patient-centered care.

Using NC*Notify comes with many benefits, including:

  • NC*Notify notifies providers when their patients have received care in other care settings (e.g., hospitals, primary care practices, specialty practices, county health departments and behavioral health facilities).
  • NC*Notify gives providers insights to check in with patients after events and to schedule follow-up appointments for clinical interventions.
  • NC*Notify reduces avoidable hospital readmissions and improves coordination across the care continuum.
  • NC*Notify helps providers achieve financial goals under value-based care contracts.
  • NC*Notify helps providers comply with state and federal quality initiatives, including meaningful use and promoting interoperability.
  • NC*Notify allows for increased provider reimbursement for rapid follow-up to a patient’s hospital admission (if billed per correct CPT codes).

NC HealthConnex is pleased to offer a robust set of notification features and updates in the latest version of NC*Notify. Advanced alerts that are available include:

  • High utilizer alert – This notification helps providers identify frequent visitors to emergency departments (two or more visits in 90 days or less) or patients at high risk for readmission (four or more admissions in 12 months).
  • Dental alerts – Providers are alerted when patients visit the emergency department for dental care.
  • Care team change alert – Providers are informed when a new organization has subscribed to their patient.
  • Diabetes diagnosis alert – Providers are alerted upon a new diabetes and/or pre-diabetes diagnosis for patients they are monitoring.  
  • Chronic care management alert – Providers receive an alert when a patient meets the Centers for Medicare and Medicaid Services' chronic care management services criteria.
  • COVID-19 lab results – Providers are alerted when a COVID-19 lab test is performed, allowing them to react to positive cases in a timelier manner.
