Use the searchable list below to find a provider in your area who has an executed agreement with NC HealthConnex, or view an interactive map of providers.

• To find a provider by name or city, use the search box at the top left of the table. You can also filter by participant type or status.
• By default, the list shows 10 participants on a single page. To see more participants, click on the "Load More" button at the bottom of the list.
• Participants' status is defined as:
  • Development underway – connection build
  • Intake – on hold
  • Live in production – sending data to NC HealthConnex
  • Live not receiving – live connection but not sending data or sending limited data
• You can also export the participant list to Excel.
• If you see incorrect information about your facility, please send a message.

This page is updated between the 15th and 18th of each month and was last updated July 15, 2026. Information within the list is valid through July 1, 2026.
