Opt Out

The N.C. General Assembly has created a way for patients to prevent their information submitted to NC HealthConnex from being shared between participating health care providers, called “Opt Out.”

You have the right to opt out of having your information shared between providers through NC HealthConnex. Opting out of NC HealthConnex will not adversely affect your treatment by your physician, and you cannot be discriminated against if you do decide to opt out. If you change your mind about participating in NC HealthConnex, you can opt back in at any time by completing a new form. Forms are available on our website and may be available at your provider's office.

Read more about opt out and find downloadable forms.
