NC HealthConnex Participants

Use the searchable list below to find a provider in your area who has an executed agreement with NC HealthConnex, or view an interactive map of providers. Please note:

• To find a provider by name or to filter by participant type, city or status, use the search box at the top right of the table.
• By default, the list shows 50 participants on a single page. To change the number of participants you're viewing, click on the dropdown arrow that says "Show entries" at the top left of the table.
• The default shows the participants in alphabetical order, but you can sort by any column using the up and down arrows next to each column's title.
• To navigate to the next page, scroll to the bottom of the table and click the page number, or "Click next" at the bottom right.
• To export the list of the participants, click the "Export to Excel" button located at the top left of the table.
• If you see incorrect information about your facility, please send a message.

This page is updated between the 15th and 18th of each month and was last updated November 15, 2024. Information within the list is valid through November 1, 2024.

Note: NC HealthConnex participants also include members of the Patient Centered Data Home national network, an initiative of Civitas Network. Find an updated list of PCDH participants in 26 states connected to NC HealthConnex.
