N.C. Diabetes Specialized Public Health Registry The N.C. Diabetes Specialized Public Health Registry, developed by the N.C. Health Information Exchange Authority and the N.C. Division of Public Health, is available for population health purposes. The N.C. Diabetes Registry is a tool for tracking the clinical care and outcomes of the patient population in North Carolina diagnosed with or presenting signs or symptoms of diabetes. Full participants in NC HealthConnex are eligible to participate in the registry by completing and signing the N.C. Diabetes Registry Form. Data submitted to NC HealthConnex will be included in the Diabetes Registry, as appropriate. No additional data submission from participants is required. The N.C. Diabetes Registry supports attestation for Meaningful Use Stage 3 and Modified Stage 2 for eligible hospitals, eligible critical access hospitals and eligible professionals, as well as Medicare Quality Payment Program Advancing Care Information for eligible clinicians. NC HIEA will provide health care organizations with specific emails that need to be distributed to each heath care provider participating in the Meaningful Use disease registry objective to show confirmation of participation for future audits.
